Well this is probably the first of the comics that don’t work for me. :( Sorry, but from the outfit it’s totally predictable from the start. Maybe if glasses guy wore a typical western outfit in the first 3 panels it would have worked and everyone would still get it.

I’d suggest to somehow fade the clothes to a dentist’s outfit in panel 2 and 3, so the reader would be all “WTF is happening?!”, and then get it in panel 4.
